With gray paper cover hinged at top with black tape. Some minor discoloration to cover with label to lower right corner with "NEVER USED" in black ink. Composed of tissue overlay over various cut papers secured to backing board and with black and blue ink lines. The tissue overlay secured along one side with masking tape, the opposite corners with masking tape squares, no longer adhered to backing board. The backing board with yellowed dry adhesive from tape squares.
