Suffering from ‘fog, destroying certain parts of the male organism’, but still coming to see Roshchin.

‘And yet, Capitan, I am on my way to you! … I am setting off tomorrow: my doctors informed me that this “fog”, destroying certain parts of the male organism, has only deleterious effects on people of lower ranks. So what am I worrying about? To you, however – it is even beneficial to be struck in that part by “fog”: it is long overdue! The heat in Paris is just hellish. I visited Biarritz – there is not a single room to be found! Seriously – just wasted my time doing it. I embrace you’.

Bunin’s euphemistic ‘fog’ is hard to interpret, but may refer to the hemorrhoidal bleeding from which he suffered in later life, leading to severe anemia.
