With vertical median line in ballpoint pen. With possible tape remains along extreme edges. Scattered areas of light surface staining throughout. With rough cut left, right, and upper edges. With horizontal lines in ballpoint pen tracing the profiles of painted forms. With four pinholes to the extreme corners. With additional scattered pinholes and micro-punch holes throughout. With three punch holes, ⁵⁄₁₆ in. in diameter, along right edge and one at upper left.

Reverse: Overall with scattered remains of watercolor and graphite. Labeled in ballpoint pen 'July 28, 1971 / BY ZUZEK / 4 SCREENS / 4 POS.
